Keep intake forms short. 2 to 4 questions maximum for a discovery call. Each required field adds friction that can reduce bookings.
Arriving at a discovery call without context is a wasted first five minutes. Intake questions collect that context at the point of booking, automatically mapping answers to the contact's CRM record so you have everything before you connect.
Steps
Open the calendar in edit mode
Go to Calendars, find the calendar, and click Edit.
Navigate to the form section
Find the Forms or Custom Fields section in the calendar settings.
Add your questions
Click Add Field for each question. Choose the type: Text, Text Area, Dropdown, or Checkbox. Common useful questions: biggest current challenge, how they heard about you, have they worked with a coach before.
Mark required fields
Toggle Required on fields that must be answered before the booking confirms. Keep required fields to 1 or 2 maximum.
Map answers to CRM fields
For each intake field, map the answer to a custom contact field. Answers then populate the contact record automatically and can be used in workflows and personalisation.
Test with a booking
Complete a test booking and fill in the intake form. Open the resulting contact record and confirm the answers mapped correctly.
- Too many required questions. Every required field is friction. Make only truly essential fields required.
- Not mapping answers to CRM fields. Answers that only live in the booking form cannot be used in automations or personalisation.
- Asking for information you already have. If the contact is in your CRM, do not ask them to repeat what you already know.
